Subjective the subjective section of the notes is where you will record the questions you ask and the clients answers. This section is not backed, but to record the observation of the owner or yourself. These notes are not exact or factual and not diagnosis. this can be written out in a narrative form if desired. The answers to any questions you ask should be recorded in the client's own words and based on or influenced by personal feelings, tastes or opinions. Examples of some questions might be as the pet exhibited any abnormal coughing, sneezing or vomiting.
Abnormal in this case is not a cat vomiting a hairball. It is abnormal if they vomit profusely for days on end. This is where your judgment comes in for what is normal and abnormal for the patient has the pet been eating as they normally do, what brand of food does a pet eat and how much and how many times per day any change in level of thirst has a pet exhibited any signs of lethargic or abnormal behavior? Has the pet been excessively itchy at all? If so, have the owner point out where on the pet ears base of tail etc. are the pets urination habits normal more or less frequently than usual small amounts straining to urinate dark color of urine exceptionally strong smell are the pets bowel movements normal regular solid diarrhea nothing strange seen in the feces is the pet on regular monthly flea and tick medications.
Are they on medications or supplements for anything else? Always confirm the dosage amounts of every medication and is there anything else the client would like for the doctor to be aware of
