Hello, and welcome to the lecture on the exciting world of paperwork. So there are five different pieces of paper that I use when I work with clients. Some of them are required by law and some of them aren't not just share those with you, you'll have to check the laws in your state or the area that you work to see if they're applicable for you. But the first one is, is a registration. And so I live in the state of Washington in the United States and as a hypnotherapist I am required to register with the Department of Health so in the areas where you live, there's probably most likely a government agency that regulates therapists and psychologists and whatnot so select to pay money to be on this registration. It's an annual fee I get a little registration form in the mail and all that registration does is it puts me in a pool of names where people can go to the state if they need to, to file a complaint.
So they're is no diploma or credential that is involved to be registered. But it is a requirement for the state that I live in. The second piece of paper is called a client disclosure form. And so what I've done with that one I attached that, attach that with my intake form. So I have a very, very simple intake form where I just take their name, their contact information, and if they want to be on my email list, you can take more information if you want. And then I attach to that, what's called a client disclosure form, which was a required by the state of Washington, that I give this information to people before I do a session with them.
So I attach that to my intake form. They read the information and they we both sign it and then I keep that on file with me. I don't send that home with them. And then the next piece of paper required by the state of Washington is called a professional conduct. And so what that is, it's a piece of paper that tells your clients that you've given them information about the state of Washington, how they can go about filing complaint with you. And so there are rules and regulations and ethics regarding behavior as a therapist, and we'll get into that a little later.
But you need to send that piece of paper home with them if you're working in person. And then the last one is required by the federal government in the United States. So when people come see me again, one on one, I will send a home the piece of paper regarding privacy, and that essentially states that I don't share their information with anyone. And again, the state of Washington is also the same. I'm not allowed to share my information with anyone, all sessions are absolutely confidential, and the only way I can release information about my clients is through a court order. So, you know, when you go home at night, or after you've worked with someone, you're going to want to talk to your spouse or whatever about maybe this particular client that you've worked with.
Don't do it. Don't even mention their name or what you've done with them. So you know, you have to have your legally and morally bound by keeping all your senses confidential. So you're like the priest or priestess? Don't say a word. And then the last one is just notes that I keep for myself after the end of each session.
So you can make up whatever you want. But essentially, I just will have some their name, where they preferred from progress they've made since the last session. And then I have a little tick box where I'll tick off whatever type of technique I used. And then real important part is to keep track of the money. So you want to see how much they paid for that session. So I do package deals and so people will buy five at a time.
And I'll make sure that I say it's number three of five or whatever because, you know, sometimes I go a month without seeing a client and then I go home, Alright, number three, or number four, I can't remember you know, so you want to make sure the money is all straight and clear. So so that's it on the paperwork. It's not too complicated. I try and keep it as simple as possible. And in the summary, I will have PDF files that you can download to use as templates and get your own ideas for the necessary paperwork that you need. So, enough of that.
